Scan the QR code to access the website. If this is your first time using the service, click Register to create a new profile with a password and begin editing your personal information. Click Save. Start accessing your health history.

Each QR code is unique, and you can register as many codes as you need within the same profile.

Is my personal information safe? Is there any cost?

Your personal information is securely stored on Vital+ID’s protected servers in the United States, recognized for their high level of security. We are committed to safeguarding your data with industry‑standard security measures.

Currently, we offer free storage services to all of our users.

What should I do if my bracelet or tags are damaged or lost?

1. In your personal Vital+ID profile, you can view the codes you have registered.

2. You can delete them easily, and they will no longer be linked to your profile or medical record.
Replacement Policy:

Quality issues: We offer a free replacement for products with quality problems within the warranty period. Certain restrictions apply.
Damage/loss: If a product is damaged or lost, a replacement fee will be charged.

Solution to the bracelet plate reflection

Our bracelet features an engraved steel plate, which can sometimes create a reflected image when scanning the code. To avoid this, simply adjust the lighting and shadow by moving your wrist up or down and scan directly. With our medical ID bracelet, you can rest assured that your important medical information is always with you.
